We've set the deadline for all team submissions on Sept. 21 but up to now not all teams have completely submitted the requirements. Please see the Checklist we've made re your submissions and promptly email us back those that you have not submitted.

SUMMARY OF REQUIRED FORMS (may be downloaded from this site. See other Blog entries):
- Registration Forms - Part 1 - Player Details (submit 1 for every player on your team)
- Registration Form - Part 2 - Marker Details (submit 1 for the whole team) - not required for Divisions 3 & Corporate
- Team Confirmation Form (submit 1 for the whole team)
- Team Profile - a short write-up about your team (history and accomplishments). This will be read during the Parade of Teams on Opening Day
- Hotel Bookings of Int'l Teams - hotel rooms booked, inclusive dates of reservation, people occupying each reserved room
- Flight Details of Int'l Teams - Arrival and Departure details of players, crew, and guests arriving with the team
- Names of Crew (2) and Guests to be issued IDs/Passes
